Job summary

Hawthorne Center for Rehabilitation and Healing of Ocala is seeking a Social Services Assistant to support patients, residents, and families by addressing social, emotional, and psychological needs in a healthcare setting.

You will help ensure access to appropriate care and resources, collaborate with interdisciplinary teams, and contribute to quality of life for patients while maintaining thorough documentation and effective communication with families.

Qualifications

• Bachelor’s degree in social work or related human services field preferred.
• Associate’s degree in social services or related field with some social service experience.
• Two years of supervised social work experience in a healthcare setting preferred.

Responsibilities

• Assist in addressing patients’ social and emotional needs.
• Support creation and implementation of care plans.
• Provide referrals to community resources and coordinate discharge planning.
• Maintain accurate documentation and advocate for patient needs.
• Collaborate with healthcare staff and facilitate communication with families.
