Job summary

Ivy Hill Post Acute in Pennsylvania seeks a Director of Social Services to lead our social services team and support residents and staff with compassionate, patient-centered care. You will oversee assessments, care planning, and coordination to meet emotional and social needs of residents.

Responsibilities include collaborating with the interdisciplinary team, participating in admissions and discharge planning, maintaining accurate records, and ensuring survey readiness.

Qualifications

• Two years of social work experience in a long-term care facility.
• BSW or Bachelor's degree in a related field.
• Experience using PointClickCare or other health information management system; proficiency using Microsoft Word, Excel and Outlook.
• Excellent interpersonal and listening skills, patient/resident focused, the proven ability to set priorities, problem solve and manage conflict.

Responsibilities

• Support the Social Services Director with department planning and organization to meet resident needs.
• Develop rapport with residents, families, and staff to maintain social services programs.
• Participate in admission and discharge planning, conduct assessments and care planning.
• Enter assessments and notes into PointClickCare and department logs; ensure survey readiness.
• Investigate complaints and assist in resolving issues; discuss purchases when needed.
