Job summary

Diversicare is seeking a Social Services Assistant to support pre-admission and admission processes and assist with Medicaid ICP approvals. The role involves interviewing clients, helping families apply for Medicaid, and coordinating with the Social Services Director on resident histories and post-discharge planning.

The position emphasizes communication with residents, families, healthcare professionals and facility staff, plus maintaining documentation and care plans across interdisciplinary

Qualifications

• High school diploma or equivalent; Bachelor's in Social Work or Human Services preferred.
• At least 1 year of supervised social work experience in health care.
• Experience or knowledge of ICP qualifications and application process preferred.
• Ability to work with a variety of people in diverse settings.
• Able to make decisions based on evaluations of client histories.

Responsibilities

• Assist in pre-admission and admission processes as back-up to Admissions.
• Interview clients and families to determine Medicaid status and needs.
• Assist families in applying for Medicaid ICP approvals and attend annual reviews.
• Help track Medicaid applications/approvals with the facility bookkeeper.
• Assist Social Services Director with interviewing residents and families.
• Document progress and update care plans at interdisciplinary conferences.
• Provide one-on-one counseling or group discussions for residents and families.
• Maintain effective communication with residents, families, and staff.
• Handle situations involving protected health information as needed.
