Job summary

Avalon Health Care Inc. is seeking a Social Services Assistant for Avalon Care Center Tacoma. This part-time role, Monday through Friday, offers a pay range of $26.00–$28.00 per hour.

You will support residents and families during admissions and care planning, coordinate with the interdisciplinary team, and ensure resident rights and well-being are upheld. Qualifications include healthcare experience in a skilled nursing setting, strong communication, teamwork, and solid organizational skills.

Responsibilities

• Support residents and families during admission, care planning, and discharge processes
• Help residents adjust to changes and transitions during their stay
• Assist with psychosocial assessments, care plans, and resident documentation
• Advocate for resident rights and promote resident well-being
• Coordinate with interdisciplinary team members to support quality outcomes
• Build positive relationships with residents, families, and community resources
