Genesis Healthcare in Orono, ME seeks a Social Services Assistant to join our team. You will work with patients and families to provide support, resources, and compassionate guidance as part of a caring long-term care environment.

Under supervision of a social worker, you will coordinate transfers, respond to concerns, and help residents adjust. This role requires an associate degree in human services and state certification as needed.

Qualifications

• Associate degree in human services with a minimum of two years' experience in long-term care.
• Must possess any certification required by state regulations.

Responsibilities

• Collaborate with patients/residents and their families to provide support and resource information.
• Respond to issues identified by patients/residents and their families.
• Facilitate patient/resident transfer within the nursing center to ensure a seamless transition and patient/resident adjustment and document accordingly.
• Participate in the health care decision-making process within the nursing center under direction of a qualified social worker and/or qualified consultant.
