Genesis HealthCare, Inc. in Albuquerque, NM is seeking a caring professional to support patients and families within our long-term care center. The role focuses on linking residents with resources and ensuring smooth transitions with the guidance of the social work team.

Responsibilities

• Participate in the health care decision-making process within the nursing center under direction of a qualified social worker and/or qualified consultant.
