Job summary

Burlington Health and Rehabiliation Center in New Jersey is seeking a Social Service Assistant to join our social services department. You will work closely with residents to ensure quality care and improve their lives.

This full-time role is Monday–Friday daytime hours. Duties include assessing residents' social and emotional needs, planning admissions and transfers, participating in care plan meetings, and coordinating with finance and discharge planning to ensure a safe return home.

Qualifications

• Experience in long term care or SNF.
• Experience working with a diverse aging population.
• Social work setting experience is preferred.

Responsibilities

• Maintain and improve residents' ability to manage daily physical and psychosocial needs.
• Identify individual social and emotional needs.
• Plan admissions, community re-entry or transfers.
• Join interdisciplinary care plan meetings to address social/emotional needs.
• Coordinate with finance to ensure residents understand and have required documents.
• Assist with discharge planning and arrange DME or home care for a safe return home.

Job description

Burlington Health and Rehabiliation Center is seeking a Social Service Assistant to join our social services department. As a Social Service Assistant, you will work closely with our residents to ensure quality care is provided and improve the lives of our residents.

This is a full time position working Monday - Friday day hours.

Some duties may include:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the communit

Job Qualifications:

• Experience in long term care or SNF
• Experience working with a diverse aging population
