Job summary

Avalon Care Center Honolulu is hiring a Social Services Assistant to join our full-time team. The role involves developing social histories, psycho-social assessments, discharge plans, and completing MDS and Care Plans for new admits, while orienting residents to the facility.

The position includes Monday–Friday daytime hours at our Honolulu location, with a starting pay of $25.00 per hour. Full-time employees receive a range of benefits and eligibility for retirement and insurance options.

Qualifications

• Bachelor’s Degree in Social Work or related field is required.
• Health care experience is preferred.
• Must meet any state and/or Federal requirements if applicable.
• Basic understanding of computer technology.
• Must be able to read, write and follow oral and written directions in English.
• Must relate positively to residents and families and work cooperatively with other employees.
• Must pass drug screening, criminal background investigation, and reference inquiry.

Responsibilities

• Develop a comprehensive social history assessment, a psycho-social assessment, discharge plan, and complete the MDS and Care Plan for new admits.
• Orient and assist new residents in adjusting to the facility.
• Assist residents and families with adjustment to changes, including room changes.
• Assist families with the engraving and/or marking of residents’ personal items.
• Review residents’ rights with residents and staff, and attend to various personal needs of the residents.
• Document progress notes related to each resident’s care plan when necessary and within policy time-frame.

Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.
