Description

The Kentucky Department for Community Based Services (DCBS) is a multi-faceted agency that provides services and programs to enhance the self-sufficiency of families; improve safety and permanency for children and vulnerable adults; and engage families and community partners in a collaborative decision-making process. The Division of Guardianship makes personal and financial decisions for individuals adjudicated dependent and appointed by the District Courts to the Cabinet for Health and Family Services. The division provides individualized decisions based on the best interest, most appropriate and least restrictive alternatives for those appointed to guardianship. The department works with a very diverse population, community resources, the court system, community partners, and family members of individuals appointed to guardianship and other Cabinet and department staff in a collaborative relationship. The Department is looking for an individual with the passion to help others.

This position, Social Service Clinician II, provides professional intensive social work services for complex guardianship cases. Provides mentoring and training reinforcement services for entry-level social workers.

The successful candidate should be professional, polite, and able to work with a diverse population. The candidate must be able to deal with difficult situations in a calm, respectful manner. They should be responsive, willing to meet each challenge directly, and able to ask for assistance or guidance when needed. The successful candidate must have the drive to meet the needs of those we serve.

Responsibilities Include But Not Limited To

  • Provides guardianship services for disabled persons to include making all medical, personal and/or financial decisions for clients. Provides intensive social work services for complex cases including those with no resources for basic needs. Responsible for face-to-face visits with client to assess needs and living conditions and assist with community resources.
  • Responsible for preparation of court reports and monitor conditions as set by court appointments. Works to provide intensive social work services to those complex cases appointed by the court including those with severe mental illness and those with no resources for housing or personal needs.
  • Serves as technical expert to the office supervisor, staff members, court officials and community partners. Including mentoring new or less experienced staff and training staff on policy, process and operations through job shadowing, one on one discussions and technical assistance. Serves as interim supervisor as needed.
  • Works as a member of the service plan review team to establish a plan of care. Ensure unified service provisions including monitoring of clients activities of daily living, and community involvement. Works with staff to ensure payment of bills personal needs allowance and to establish and maintain benefits including Medicaid, food stamps, pensions, federal and other eligible benefits.
  • Enters confidential data into the Kentucky Guardianship Fiduciary Information System and maintains confidential records.
  • Provide expert court testimony and work with referral sources to provide education regarding Guardianship to ensure the referrals are appropriate prior to petitioning the court for appointment.
  • Other duties as assigned.

Minimum Requirements

EDUCATION: A master's degree in social work, sociology, psychology, marriage and family therapy or a related field.

EXPERIENCE, TRAINING, OR SKILLS: Must have two years of professional social work experience.

Substitute EDUCATION for EXPERIENCE: NONE

Substitute EXPERIENCE for EDUCATION: A bachelor's degree supplemented by four years of professional social work experience will substitute for the primary education and experience requirements.

SPECIAL REQUIREMENTS (AGE, LICENSURE, REGULATION, ETC.): If employed by the Cabinet for Health and Family Services or the Department of Juvenile Justice, an employee in this job classification must furnish to the appointing authority within one month of employment, documentation that he/she possesses a valid driver's license. Must maintain any required licensure(s), certification(s), or other credentials for the length of employment in this job classification. Employing agency is responsible for ensuring employee possesses and maintains required licensure(s), certification(s) or other credentials.

Working Conditions

Performs job duties in an office/facility setting and visits clients and/or families at their residence. Provides emergency services on 24-hour basis, including accepting referrals and completing investigations at any hour without prior warning. As a result of enforced intervention, the worker runs the risk of physical harm.
