Project Summary:

TheNew York State Department of Healthhas establishedSocial Care Networks(SCNs)as part of the1115 Waiver Demonstration Amendment,and Forward Leading IPA (FLIPA) is the SCN Lead for the Finger Lakes Region. This region is comprised of 14 counties, including Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, and Yates. The SCN’s aim is to enhance the delivery of social care services toMedicaid membersby coordinating efforts amongcommunity-based organizations (CBOs) and other health care partners to create a more resilient, flexible, and accessible social care system that reduces health disparities and advances health equity. The SCN will collaborate with CBOs and other health care partners, leveraging shared data and technology to coordinate social care services for Medicaid members to improve access, ensure reliable and timely referrals, streamline and track navigation and completed referrals in closed loop systems and enhance collaboration between social care service providers and other regionalpartners.

Job Summary:

The SCN Communications Manager is responsible for coordination, development, and oversight of internal and external communications related to the SCN, engagement with vendors for Website, Learning Management System (LMS), and Customer Relationship Management (CRM) support. The SCN Communications Manager is responsible for collaborating and implementing a communication strategy to support awareness, utilization, and engagement with current and potential partners as well as Medicaid-eligible individuals to support screening and navigation to social care services throughout the Finger LakesRegion.

Duties/ Responsibilities:
  • In collaboration with FLIPA SCN leadership, develop a FLIPA SCN brand platform
  • In collaboration with FLIPA SCN leadership, design and execute a communication strategy that utilizes a broad, cost-effective range of platforms to grow awareness and brand recognition across a variety of audiences in the Finger Lakes region
  • Collaborate with the Director of Health Equity and Community Impact to develop and implement a comprehensive health equity communication plan, ensuring seamless integration across all FLIPA communications.
  • Develop and share social media communications across applicable platforms (e.g., LinkedIn, Facebook, Instagram) to engage both CBOs and eligible Medicaid recipients.
  • Monitor success of media campaigns and activity to identify and maximize effective communication strategies
  • Regularly update and refresh FLIPA/FLIPA SCN websites with relevant content that meets state requirements and leadership-identified goals
  • Provide support in preparing materials for communications to the FLIPA Board, SCN governing body and other committees to communicate key messages related to SCN communication and marketing activities.
  • In collaboration with team members, proactively write/proof/edit press releases and similar network- and public-facing correspondence
  • Design and edit graphics for print or web as needed
  • Contribute to development of training materials that engage diverse audiences
  • Other duties as assigned.

Education & Experience:
  • Bachelor’s degree in marketing, communications, public relations, graphic design or closely related field or significant equivalent work experience required
  • Minimum 2 years of relevant work experience required.
  • Experience in website development and CRM preferred.
  • Experience and/or working knowledge of CBOs, networks/collaboratives, and NYS health system in the Finger Lakes Region preferred

  • Fluency in Adobe Creative Cloud or similar
  • Fluency in Social Media Platforms and working understanding of analytics
  • Ability to prepare and present proposals, reports, updates, summaries, etc.

  • Ability to work as part of a team and follow established branding, communication, and approval guidelines, while communicating ideas for improvement
  • Ability to build constructive working relationships with internal and external partners
  • Strong interpersonal skills, independently resourceful and thrives in a fully remote environment
  • Takes responsibility for the quality and timeliness of work and achieves results with little oversight
  • Proficient with MS Office and various business software

  • Salary is commensurate to education and experience with range of $60-80k
