Provide quality, clinical services to patients in various community settings, following organizational philosophy, policies, and standards. Perform admissions and case management in accordance with PHH policies, utilizing electronic documentation and interdisciplinary communication. Deliver skilled nursing care including assessment, teaching, treatment, documentation, and care coordination, emphasizing high-quality customer service and financial awareness.

Key Responsibilities
  • Ensure care is provided with effective utilization and accepted standards for quality outcomes.
  • Accurately perform initial assessments and visit notes reflecting medical necessity, patient status, services provided, vital signs, education, and comprehension, ensuring proper reimbursement and high-quality care.
  • Develop and update the plan of care to meet patient needs, safety, and outcomes, with goals being patient-specific and timely.
  • Communicate patient information and changes to physicians promptly and document accordingly.
  • Reconcile medications accurately at start of care, return, and transfer, reviewing at each visit.
  • Order supplies and manage wound supplies in consultation with WOCN.
  • Establish and communicate patient schedules, adjusting as needed to meet patient and agency needs.
  • Adhere to Point of Care data submission standards, including timely encounter documentation.
  • Maintain software updates and prepare patient information for communication with relevant departments.
  • Make referrals to other disciplines and community resources to optimize patient safety and outcomes.
  • Identify patients for multidisciplinary case conferences post-start of care.
  • Provide patient education in preferred language and format, documenting responses and effectiveness.
  • Initiate discharge planning early, involving primary physician, family, and community resources, and document accordingly.
  • Ensure all consent and notification forms are completed within 24 hours, with services delivered timely and according to patient needs and preferences.
  • Include patients and families in care planning, documenting consent to changes.
  • Maintain high standards of care through adherence to protocols, communication, coordination, and documentation.
  • Perform complex assignments supported by competency documentation, including high-tech visits, end-of-life care, and complex wounds.
  • Attend staff meetings, training, and educational sessions regularly, supporting clinical initiatives and mandatory education.
Education/Degree Requirements

Graduate of an approved School of Nursing

Licensure, Certifications or Registration

Current Massachusetts license as Registered Nurse

Experience
  • Minimum of one year of nursing experience in acute or sub-acute care required.
  • Less than 1 year of experience in RN Residency Program.
  • Prior home care experience preferred.
  • Travel within MGB Home Care's area required.
  • Proficiency with computer software and knowledge of handheld devices preferred.
  • Strong interpersonal skills and ability to work with diverse populations.
  • Effective communication skills in writing, verbally, and electronically.
  • Ability to organize, prioritize, and adapt to changing situations independently.

Primarily working in patients' homes, with potential exposure to blood, body fluids, and chemical hazards. Attend meetings in various external venues.
