Pay Plan Title: SOC Family Partner
Working Title: Community SOC Family Partner
Location: Remote within North Carolina's Southern Region, including counties: Bladen, Brunswick, Columns, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland.
We manage mental health, substance use, traumatic brain injury, and developmental disability services across 46 counties in North Carolina. Our mission is to support individuals and families in building healthier lives.
The Community SOC Family Partner educates parents and caregivers on navigating child-serving systems and connects families to community resources. You will provide whole-person care, focusing on physical health, behavioral health, and social determinants, to improve outcomes for youth and families.
High School Diploma or GED required, with at least four years of experience as a primary caregiver for a child or youth receiving Medicaid or state-funded behavioral health services.
Preferred experience includes two years working directly with individuals with behavioral health or intellectual/developmental disabilities.
Valid driver’s license required; Certified Peer Support Specialist preferred.
