Job summary

Keys to Change is seeking a SOAR Specialist in Phoenix to improve access to SSI/SSDI for homeless individuals, especially those affected by substance use and mental health conditions. The role focuses on identifying eligible clients and guiding them through the SSI/SSDI application process.

You will collaborate with SSA and partner agencies, track data, and prepare medical summaries to strengthen applications. This position offers office-based work with local communities and meetings.

Qualifications

  • Bachelor’s degree in human services or related field.
  • Familiarity with SSA processes for applying for benefits.
  • Experience coordinating with DDS/SSA and medical records to complete SSI/SSDI applications.
  • Spanish language skills a plus.
  • Experience working with homeless populations.
  • Strong written and verbal communication skills.

Responsibilities

  • Identify clients eligible for the SOAR program.

  • Participate in weekly/monthly SOAR coordination meetings.
  • Liaise with SOAR technical assistance specialist.
  • Assist Housing Program Manager with SOAR presentations and trainings.

Summary

The purpose of SOAR is to improve access to SSI/SSDI for homeless individuals—especially those affected by substance abuse and mental health conditions, who often experience difficulty in accessing such benefits.

What You Will Do to Help End Homelessness
  • Identify clients that are eligible for the SOAR program.
  • Provide intensive assistance through the application process.
  • Collaborate with partner agencies including the Social Security Administration.
  • Track and enter data as it relates to SSI/SSDI applications.
  • Write medical summary reports to support SSI/SSDI applications.
  • Communicate with clients SSA, Medical Records Departments, and other agencies to complete the SSI/SSDI application process.
  • Participate in weekly/monthly SOAR Coordination Meetings.
  • Liaise with SOAR technical assistance (TA) specialist.
  • Assist Housing Program Manager with SOAR presentations and trainings.
You're an Ideal Fit If You Have
  • Bachelor’s degree in human services field or Public Administration or related discipline or equivalent in experience.
  • Familiarity with the Social Security Administration and processes for applying for state and local benefits and entitlement programs.
  • Experience working with DDS, SSA, Medical Records Departments, and other agencies to complete the SSI/SSDI application process.
  • Strive for innovation and think outside the box.
  • Experience working in areas of crisis with vulnerable populations.
  • Multilingual (Spanish is a plus).
  • Personal Experience with Homelessness.
  • Have experience working with populations experiencing homelessness.
  • Superior written and verbal communication skills (Spanish a plus).
  • Demonstrated ability to communicate effectively and professionally with staff, clients, and other external contacts.
  • Ability to handle sensitive material, maintaining the highest level of confidentiality and displaying sound judgment.
  • Valid Driver's License/Clean 3-year MVR.
  • Mobility sufficient to conduct regular duties within a normal office environment.
  • Ability to lift up to 20 pounds in order to safely manage office supplies and equipment.
