Job summary

Sweetwater Care is seeking a licensed Physical Therapy Assistant to assist in rehabilitation and mobility restoration for residents recovering from serious illness or injury. Under the supervision of a Physical Therapist, you will help develop and implement therapy programs aimed at restoring functional movement and balance.

The role emphasizes documentation, adherence to Plan of Care, and coordination with interdisciplinary teams to ensure high-quality patient care and compliant billing.

Qualifications

• High School Diploma required.
• Requires valid Physical Therapy Assistant license or certification in the state of practice, or proof of license eligibility.

Responsibilities

• Provide support care to residents seeking short-term/transitional care with the goal to recover and rehabilitate from serious illness, injury or medical event, regaining their own best possible level of functional ability, well-being and independence.
• Assist in providing appropriate aspects of quality patient care under the supervision of licensed therapists.
• Document all services provided to ensure accurate and timely billing of therapy services.
• Ensure that patient care follows the established Plan of Care, the organization’s guidelines, meets professional standards, community needs and is in compliance with Federal and State guidelines.
• Under the supervision of a Physical Therapist, identify and develop a comprehensive plan of care; develop and implement individualized therapy programs designed to restore, reinforce and enhance performance.
• Identify need for and refer to other disciplines to address the comprehensive needs of the patient under the supervision of a Physical Therapist.

Provides support services to improve strength and function, restore more efficient movement, improve flexibility and aid patients in restoring mobility and/or improving balance and gait after hip fractures and other orthopedic injuries, medical illness, or as they recover from joint replacement procedures.
