Atrium Health in Charlotte, NC, invites Physician Assistants and Nurse Practitioners to join the Adult Medical Specialty team within the Post Acute Network — Skilled Nursing Facility program in Charlotte, NC. This on-site role offers a Monday–Friday schedule with shared call rotation and both in-person and virtual patient visits supported by a Medical Assistant.

The ideal candidate will be a PA or NP with a passion for SNF/post-acute care, and experience in a SNF is preferred but not required.

Qualifications

• Physician Assistant or Nurse Practitioner
• Passion for working in a SNF/Post-Acute Care Environment
• Experience in a Skilled Nursing Facility is preferred but not required

Responsibilities

• Monday–Friday | 8:00 AM–5:00 PM
• Shared call rotation
• In-person and virtual visits supported by Medical Assistant
• Manage complex patients with ongoing follow-up throughout stay
• Collaborate with SNF team to facilitate care coordination
• Implement discharge planning to improve patient outcomes
• Signing bonus, CME, and relocation available.
