You will collaborate with facility staff, provide education to patients and families, and ensure consistent, high-quality care while maintaining thorough documentation of progress and outcomes.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable goals.
• Treating patients, addressing concerns and educating them about injury, progress and outcomes.
• Supervising Physical Therapist Assistants and ensuring high-quality care.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Complete timely and accurate documentation including evaluations, re-evaluations and daily progress notes.
