Job summary

Community Physicians in Aurora, IL, seeks a Nurse Practitioner or Physician Assistant to join our structured Skilled Nursing Facility Physiatry Program, working closely with a board-certified physiatrist. This role is active and high-impact, focused on mobility, hospital-readmission reduction, and therapy alignment within a comprehensive PM&R pathway.

You will participate in daily SNF rounding, follow standardized clinical pathways, and collaborate with an interdisciplinary team.

Qualifications

• National certification (FNP-C, AGNP, ACNP, or PA-C).
• 1–3+ years experience in physiatry, geriatrics, post-acute, or rehabilitation preferred.
• Skilled Nursing Facility experience strongly preferred.
• Strong functional and musculoskeletal assessment skills.
• Ability to work independently within a structured, team-based model.

Responsibilities

• Provide comprehensive Physical Medicine & Rehabilitation services for post-acute and long-term care residents, including stroke recovery and musculoskeletal issues.
• Center work on restoring function, improving mobility, reducing pain, coordinating care, and transitions of care.
• Participate in structured daily rounding within designated SNFs, following standardized clinical pathways and quality protocols.
• Collaborate with interdisciplinary team and supervising physiatrist to optimize therapy alignment and outcomes.
