Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team. In this role, you will evaluate and treat patients in a skilled nursing facility setting, following an individualized plan of care.

You will collaborate with facility staff, develop measurable goals, educate patients and families, document progress, and supervise Occupational Therapy Assistants to ensure excellent care and outcomes.

Responsibilities

• Provide excellent care for patients in SNF setting
• Deliver excellent customer service to patients and families
• Collaborate with facility staff to ensure quality patient care
• Evaluate new patients and develop plans of care with measurable goals
• Treat patients, address concerns, and educate on injuries, progress and outcomes
• Re-evaluate progress and revise plans of care
• Complete timely documentation including evaluations, re-evaluations, discharges, and daily progress notes
• Supervise Occupational Therapy Assistants
