Job summary

1 Care Partners is seeking a Medical Director/Attending for a Skilled Nursing Facility in Tappahannock, VA. The physician will provide comprehensive medical care, lead clinical decision-making, and collaborate with NPs, nursing staff, and other professionals to ensure high-quality resident care.

Active Virginia MD/DO license with board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics is required.

Qualifications

• Active MD or DO license in Virginia.
• Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics.
• Experience in Skilled Nursing Facility or geriatric medicine preferred.
• Strong clinical assessment and diagnostic skills.
• Ability to work independently while collaborating with a multidisciplinary team.

Responsibilities

• Perform comprehensive medical assessments and develop individualized treatment plans.
• Diagnose, manage, and treat acute and chronic medical conditions.
• Provide ongoing monitoring and follow-up care for residents.
• Collaborate with interdisciplinary teams to enhance patient care and quality outcomes.
• Educate patients, families, and staff on disease management and preventive care.
• Ensure compliance with regulatory and documentation requirements.
• Offer clinical guidance to Nurse Practitioners and other healthcare staff.
