Job summary

Centers Health Care is seeking an Attending Physician (MD/DO) for a skilled nursing facility in Thomaston, CT. The role offers a competitive salary, malpractice coverage, and a path for growth within a physician‑led SNF-focused group.

As a physician on our team, you will oversee admissions, ensure high-quality patient care, and collaborate with APPs to optimize treatment plans while navigating regulatory requirements and maintaining thorough documentation.

Qualifications

• Active NYS medical license (MD/DO)
• Current DEA registration in good standing
• Up to date CME requirements
• Experience in SNF, long‑term care, or managed care settings preferred
• Comfort working with a culturally and linguistically diverse resident population

Responsibilities

• Perform admissions, monthly, acute & routine visits for SNF residents, on a regulatory and as-needed basis
• Collaborate with APPs on your units
• Manage medication decisions — including narcotics and psychiatric medications — with an emphasis on reducing unnecessary polypharmacy
• Lead care planning, discharge planning, and ongoing staff education
• Support the facility through regulatory and survey site visits
• Coordinate with local hospitals to ensure continuity of care for residents
