Medical Director Services PC is seeking an Attending Physician (MD/DO) to care for skilled nursing facility residents in Staten Island, NY. This physician-led group focuses on treating patients in place, reducing hospitalizations, and maintaining strong ties to the local hospital network for continuity of care.

Responsibilities

• Perform admissions, monthly, acute & routine visits for SNF residents, on a regulatory and as-needed basis.
• Collaborate with APPs on your units.
• Manage medication decisions — including narcotics and psychiatric medications — with an emphasis on reducing unnecessary polypharmacy.
• Lead care planning, discharge planning, and ongoing staff education.
• Support the facility through regulatory and survey site visits.
• Coordinate with local hospitals to ensure continuity of care for residents.
