Ascend Home Health & Hospice in Pocatello, ID is seeking a compassionate Speech-Language Pathologist (SLP) to join our team on a PRN basis, serving patients in their homes in Bannock County. You will evaluate and treat communication, cognition, and swallowing disorders and collaborate with the care team to implement individualized plans.

The role emphasizes flexible scheduling, hands-on patient care, and meaningful outcomes with a supportive, patient-centered work environment.

Competenze

• Current Idaho SLP license required.

Speech-Language Pathologist (SLP) – PRN

Ascend Home Health & Hospice | Pocatello, ID

Ascend Home Health & Hospice is seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our growing team on a PRN basis serving patients in Pocatello, Idaho.

This position offers flexibility and the opportunity to make a meaningful impact by helping patients improve communication, cognition, and swallowing abilities in the comfort of their own homes.

Responsibilities:

• Evaluate and treat patients with speech, language, cognitive, voice, and swallowing disorders
• Develop individualized treatment plans based on patient needs and goals
• Educate patients, families, and caregivers on therapy techniques and care plans
• Collaborate with physicians, nurses, therapists, and the interdisciplinary care team
• Complete timely and accurate documentation

Qualifications:

• Current Idaho SLP license required
• Home health experience preferred, but not required
• Strong communication and organizational skills
• Compassionate, dependable, and patient-focused
• Valid driver's license, reliable transportation, and current auto insurance required

Position Details:

• PRN / Flexible scheduling
• Coverage area includes Bannock County
• Pay per visit DOE, visit rate, and mileage (dependent on experience)
