Job summary

Optimum Care SLS II is seeking a compassionate SLS Caregiver / Direct Support Professional to join our team in Rohnert Park, CA. You will provide in-home and community-based support to adults with developmental disabilities, promoting independence and well-being.

The role requires caregiving experience, valid driver’s license, and CPR/First Aid certification. A flexible schedule including weekends and overnights is preferred, with training and DBS/DMV checks as part of onboarding.

Qualifications

• Valid driver’s license & insured vehicle available for shift.
• CPR/First Aid certification.
• Negative TB test within the past year or willingness to obtain before start date.
• Minimum 1 year caregiving experience, preferably with individuals with developmental disabilities.
• Ability to pass FBI/DOJ background check and DMV records check.
• Flexible schedule, including weekends, holidays, and overnights.
• Strong understanding of person-centered care and best practices for vulnerable populations.

Responsibilities

• Provide direct care and support to individuals with disabilities in a home setting.
• Assist with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and mobility.
• Support clients with feeding, hydration, and meal preparation according to individual dietary needs.
• Monitor and document clients’ health and report changes to supervisors or healthcare providers.
• Assist with medication reminders and healthcare-related tasks with appropriate training.
• Perform light housekeeping to maintain a clean and safe living environment.
• Provide incontinence care and support with restroom routines.
• Accompany clients to medical appointments, errands, and community outings.
• Encourage and facilitate social and recreational activities.
• Advocate for clients’ rights and support their independence and dignity.
• Maintain accurate documentation of services provided.
• Attend required trainings and team meetings.

OVERVIEW

Optimum Care SLS II is seeking a compassionate and reliable SLS Caregiver / Direct Support Professional to join our team in providing high-quality supported living services (SLS) to adults with intellectual and developmental disabilities in Sonoma County.

As a caregiver / direct support professional in our SLS program, you will support individuals in their homes and communities by assisting with personal care, promoting independence, and ensuring their overall health, safety, and well-being.

RESPONSIBILITIES

• Provide direct care and support to individuals with disabilities in a home setting.
• Assist with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and mobility.
