Job summary

Trinity Rehab, LLC in Suffolk, VA, seeks a dedicated Speech-Language Pathologist (SLP) to join our Home Health team on a part-time basis. The SLP will evaluate, plan, and deliver treatment for speech, language, cognitive, and swallowing disorders under ASHA standards and physician guidance.

Responsibilities include conducting evaluations, developing treatment plans, providing direct home health therapy, monitoring progress, and educating patients and caregivers.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Active Virginia state license or eligibility.
• CCC-SLP preferred or Clinical Fellow considered.
• Strong communication and interpersonal skills.
• Home Health experience preferred.

Position Summary

Trinity Rehab is seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our growing Home Health team in Suffolk, VA.

The SLP will evaluate, plan, and implement individualized treatment programs for patients experiencing speech, language, cognitive, and swallowing disorders. Services are provided in accordance with the standards set by the American Speech-Language-Hearing Association (ASHA) and under the direction of the attending physician.

Key Responsibilities

• Conduct comprehensive evaluations of speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans
• Provide direct therapy services to patients in a home health setting
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on therapy techniques and home programs
