Job Responsibilities

• Performs Patient Assessments:
  • Accurately assesses patient’s appropriateness for home care, learning needs, and progress toward goals.
  • Collaborates with patient, family, and physician to develop the plan of care for the patient.
  • Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated.
• Maintains Safe Environment for Patients and Co‑Workers:
  • Assesses social environment and adjusts or provides education as needed.
  • Assesses for potential clinical risks and adapts program as indicated.
  • Follows universal infection control measures.
• Implements Treatment Program to Achieve Designated Goals:
  • Administers occupational therapy services per physician orders.
  • Instructs in communication and safe swallow, cognitive training as indicated.
  • Develops and instructs in therapeutic exercise program.
  • Implements program to restore function and achieve maximum functional status.
  • Provides discharge planning and referral to resources as needed.
  • Communicates with physician, nurse, and other clinical disciplines as necessary.
  • Develops/implements/evaluates plan of care based on evidence‑based knowledge.
  • Revises care plan as indicated within physician orders.
• Uses Age Appropriate Teaching Methods for Patients and Families:
  • Evaluates teaching plan and revises as necessary.
  • Documents patient education tools utilized.
• Demonstrates Compliance with Agency Policies:
  • Assumes responsibility for Case Management.
  • Demonstrates knowledge of home care regulations and insurance reimbursement guidelines.
  • Assures compliance with regulatory requirements.
  • Follows agency documentation policies.
  • Shows evidence of interdisciplinary communication.
  • Ensures appropriate utilization of services.
  • Follows scheduling process.
  • Maintains visit productivity standards.
  • Participates in Team and IDT meetings.
  • Prioritizes schedule to accommodate patient's needs.
  • Travels to patient homes and facilities as indicated.
  • Complies with paid time off request process and weekend/holiday requirements.
• Assists with Growing Agency Business:
  • Provides orientation to new employees.
  • Accepts other assignments as requested.
  • Cross trains to learn new skills.
  • Use of personal vehicle is required for home visits.

Skills
