Job summary

Enhance Rehabilitation is seeking a qualified Speech Language Pathologist to join our team as a Speech Therapist. The role involves evaluating and treating patients according to an individualized plan of care established after an initial evaluation.

In a SNF setting, you will gather information from patients, families and the treatment team, communicate progress, and implement treatment plans while monitoring patient progress and adjusting therapy as needed.

Qualifications

• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.
• Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.

• Evaluate and treat patients following an evaluation to implement an individualized care plan.
• Communicate findings and progress with patients, families and treatment team members.
• Monitor patient progress and adjust treatment plans as needed.
