Job summary

Evolve Therapy Services in Clarion, PA is seeking a Speech Language Pathologist to provide in-house skilled nursing facility services on a PRN basis. The role centers on planning, delivering and coordinating SLP services in line with CMS and professional standards, emphasizing resident-centered care.

The incumbent will evaluate residents, develop treatment plans, document in NetHealth EMR, and participate in FEES/MBS protocols.

Qualifications

• Master’s degree in Speech Language Pathology required.
• CCC required or eligible for CCC.
• Active SLP license in state(s) of practice required.
• Ongoing CE to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target list, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete compliant, medically necessary documentation in NetHealth EMR.
• Oversee and implement FEES/MBS protocols with site coordination.
• Participate in screening to ensure resident needs are met timely and with clinical excellence.
