Job summary

Evolve Therapy Services in Altoona, PA is seeking a licensed Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis to support resident care.

The role involves planning, delivering, and coordinating speech therapy, documenting in NetHealth EMR, and ensuring CMS requirements are met. Active state licensure and CCC are required, with ongoing professional development encouraged.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Must have an active SLP license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with target list and goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversight of FEES/MBS protocols with site coordination.
• Participate in resident needs screening with clinical excellence.
