Job summary

NHC HomeCare Johnson City is seeking a Speech-Language Pathologist to provide comprehensive speech and language therapy in a home-care setting in Johnson City, Tennessee. The role involves evaluating patients, developing and implementing therapy plans, and coordinating care with physicians and families.

Applicants must hold a master’s degree in Speech Pathology, have CCC-SLP eligibility or ASHA CFY, and be licensed in the appropriate state(s).

Qualifications

• Master’s degree in Speech Pathology from an approved curriculum.
• CCC-SP from ASHA or CFY eligibility.
• Licensed in the state(s) where they work.

Responsibilities

• Directs and supervises the Communication Disorders Service Department.
• Provides evaluations and therapy as ordered by the referring physician and recommends a treatment program for the physician’s approval.
• Evaluates, plans, and carries out therapy programs with goals and periodic re-evaluations.
• Participates in in-service training for HomeCare personnel and CQI studies as requested.
• Maintains patient care records ensuring third party reimbursement qualifications and state/federal requirements.
• Consults with family members, physicians and other health care professionals as appropriate.
• Participates in interdisciplinary conferences to maintain continuity of patient care.
