Job summary

Harvest Crossing Post Acute in Manteca, CA is seeking a Speech Language Pathologist (SLP) for full-time roles. The position offers opportunities for graduates, CFYs, or experienced SLPs to support residents in a SNF setting.

Qualifications

• Graduating or recently graduated in Speech Language Pathology; Clinical Fellowship Year candidates welcome
• Must have or be eligible for California licensure, registration, or authorization required for the applicable level of practice
• CPR certification required; may obtain before or during onboarding
• Strong communication and documentation skills are essential
• Experience in Skilled Nursing Facility valuable but not required

Responsibilities

• Evaluate patients per physician orders and facility guidelines
• Develop individualized care plans based on evaluation findings
• Provide speech, language, cognitive, voice, and swallowing interventions
• Establish measurable treatment goals focused on functional outcomes
• Document evaluations and progress in EMR and rehabilitation systems
• Coordinate treatment activities with an interdisciplinary rehabilitation team
