Job summary

Angels Care Home Health is seeking a Speech Language Pathologist (ST) to deliver therapeutic techniques for patients with speech, language, hearing, swallowing, and cognitive disorders in patients' homes across Conroe, TX.

The role requires a Master's degree in Speech Language Pathology, a current state license, CCC-SLP, and at least 2 years of acute care or rehabilitation/home health experience, plus a valid driver's license and reliable transportation.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited college or university.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence from American Speech Language Hearing Association (ASHA).
• Valid driver’s license.
• 2 years’ experience in an acute care or rehabilitation setting; home health preferred.
• Reliable transportation with current auto liability insurance.

Responsibilities

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
• Collaborates with interdisciplinary team.
