Job summary

CommuniCare Rehab in Charleston, WV is seeking a Speech Language Pathologist for a full-time position at Charleston Healthcare Center. The role involves evaluating patients, planning and delivering therapy to improve communication, swallowing, and cognitive functions, and coordinating care with the rehab team.

You will work on an interdisciplinary team, engage with residents and families, and contribute to ongoing care plans across multiple locations, promoting resident well-being.

Qualifications

• Must have a Bachelor and/or a Master’s Degree in Speech-Language Pathology from an Accredited Program.
• Hold a valid or pending State Speech Language Pathology License.

Responsibilities

• Evaluate, review, and screen patient’s medical charts and develop a treatment plan per physician order.
• Plan, direct, or conduct rehabilitative treatment programs to improve communication, swallowing, and cognitive skills.
• Instruct residents, families, and caregivers to monitor speech and practice new skills; advise on diet and safe swallowing.
• Attend/lead interdisciplinary meetings to relay clinical findings and make rehab recommendations.
• Treat at multiple locations to serve diverse resident needs.
