Qualifications

• Licensed Speech-Language Pathologist.
• Open to applicants of all experience levels.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL impairments and develop plans of care.
• Use standardized assessments and evidence-based practices to guide interventions.
• Provide rehabilitative, skilled services per regulatory requirements.
• Administer procedures as part of the rehabilitation plan (ADL retraining, therapeutic activities, exercises, modalities, assistive devices).
• Assess progress and adjust treatment in the clinical record.
• Evaluate outcomes and refer as needed based on results.
• Provide counseling to patients and families about the disorders.
• Communicate progress with supervisor and health team.
• Prepare comprehensive discharge summaries.
