Job summary

Genesis HealthCare is seeking a Skin Health Team Lead to drive skin integrity and wound management across the nursing center, partnering with the interdisciplinary team and families to ensure optimal resident outcomes.

You will lead wound rounds, oversee admissions related to skin needs, guide care planning, and support staff through training and process improvements to uphold high-quality, person-centered care.

Qualifications

• RN; or MPT/DPT with senior operations approval.
• Licensed in the state where position exists.
• Skin Health Team Lead orientation within 90 days of hire.
• Completion of Wound Basics within 90 days of hire.
• Advanced Wound Training within 1 year of hire.
• National accredited Wound Certification within 1 year of hire.

Responsibilities

• Facilitate a person-centered approach to skin and wound prevention and management.
• Coordinate and lead weekly wound rounds with comprehensive assessments and care planning.
• Lead admissions discussions for residents with skin/wound needs.
• Review new admissions/readmissions for skin integrity and coordinate interventions.
• Evaluate new skin/wound conditions within 48 hours of admission.
• Manage specialty support surfaces and lead quality improvement initiatives.
• Ensure staff competency and accurate documentation of wound care.
• Support continence and incontinence product utilization within nursing care.
• Communicate clinical outcomes related to skin and wounds.
• Provide bedside training on skin and wound care best practices.
