Job summary

Capital City Nurses invites experienced RNs to lead clinical excellence in the Montgomery County market, guiding home care teams and mentoring clinicians to improve patient outcomes. This supervisory role blends leadership with direct patient care oversight in home settings.

You will supervise, mentor and support field RNs and LPNs, conduct supervisory visits, chart audits and competency evaluations while upholding HIPAA, quality standards and comprehensive care plans.

Qualifications

• Active Maryland RN license; DC/VA a plus.
• Minimum 2 years nursing experience with supervision/mentoring.
• Strong assessment, care planning, and clinical decision-making.
• Valid driver's license and reliable transportation.
• Excellent communication, organizational and documentation skills.
• Preferred: Home Health/Skilled Home Care experience; multi-state licensure.

Responsibilities

• Conduct admissions, assessments, recertifications and change-in-condition visits.
• Develop and oversee individualized care plans with clinical teams.
• Monitor patient progress and intervene when needs change.
• Provide real-time clinical guidance and support to field staff.
• Ensure policy, standards and HIPAA compliance; EMR documentation.

Travel Requirements

• Primary coverage area includes Chevy Chase, MD and neighboring communities within approximately 20 miles
• Regular local travel required for patient visits, supervisory visits, and admissions
• Travel to other outlying service areas based on patient census, staffing needs, and business priorities is required for this role
• Mileage reimbursement provided in accordance with company policy

What You'll Do

Champion Exceptional Patient Care

• Conduct admissions, assessments, recertifications, and change-in-condition visits
• Develop and oversee individualized care plans
• Monitor patient progress and intervene proactively when clinical needs change
• Collaborate with physicians, caregivers, and interdisciplinary teams to achieve optimal outcomes

Lead and Develop Clinical Teams

• Mentor, coach, and support field RNs and LPNs
• Conduct supervisory visits, chart audits, and competency evaluations
• Provide real-time clinical guidance and problem-solving support
• Foster a culture of accountability, excellence, and professional growth

Drive Quality and Compliance

• Ensure adherence to agency policies, regulatory standards, and clinical best practices
• Maintain accurate and timely documentation within the EMR platform
• Support quality initiatives, audits, and accreditation readiness
• Uphold the highest standards of patient privacy and HIPAA compliance

Help Grow Our Skilled Services Program

• Partner with hospitals, physicians, discharge planners, and referral sources
• Support smooth patient onboarding and continuity of care
• Serve as a trusted clinical ambassador in the community

What You Bring

Required Qualifications

• Active Maryland RN License (DC and/or VA a plus)
• 2+ years of nursing experience
• Experience supervising, mentoring, or leading clinical staff
• Strong assessment, care planning, and clinical decision-making skills
• Valid driver's license and reliable transportation
• Excellent communication, organizational, and documentation abilities

Preferred Qualifications

• Home Health or Skilled Home Care experience
• OASIS experience
• Multi-state licensure (MD, DC, VA)
• Experience with AlayaCare or other EMR systems
• Associate Degree in Nursing or higher
