Job summary

Spokane Valley Health and Rehab is seeking a compassionate Social Services Director to support the emotional, social, and practical needs of our residents. This role acts as a resident advocate, helps protect rights, coordinates resources, and promotes the highest quality of life.

You will collaborate with nursing, therapy, and families, participate in care planning meetings, assist with discharge planning and transitions of care, and address social, emotional, and environmental factors

Qualifications

• Experience in social services.
• Preferred experience in skilled nursing facility.
• Ability to work effectively with residents, families, and interdisciplinary teams.
• Commitment to providing compassionate, resident-centered care.

Responsibilities

• Conduct psychosocial assessments and develop person-centered interventions.
• Advocate for residents and support resident rights.
• Maintain social services documentation and records.
• Assist residents and families with accessing community resources and services.
• Serve as a liaison between residents, families, caregivers, and community agencies.
• Participate in care planning meetings and interdisciplinary team discussions.
• Support behavior management interventions that promote resident well-being and safety.
• Assist with discharge planning and transitions of care.
• Collaborate with nursing, therapy, and other departments to support resident outcomes.
• Help address social, emotional, and environmental factors affecting resident health.

This role serves as a resident advocate, helping protect resident rights, coordinate resources, support care planning, and promote the highest quality of life.

Key Responsibilities

• Conduct psychosocial assessments and develop person-centered interventions.
• Advocate for residents and support resident rights.
• Maintain social services documentation and records.
• Assist residents and families with accessing community resources and services.
• Serve as a liaison between residents, families, caregivers, and community agencies.

Requirements

Education

• Preferred: Bachelor’s or Master’s degree in social work.
• Must have experience in Social Services. Preferred experience in Skilled Nursing Facility.

Licenses/Certification

• LCSW, LSW, or MSW license preferred.
• Must have a valid driver’s license.

Experience

• Experience in post-acute/ skilled nursing preferred.
