Job summary

Sutter Medical Group is seeking a dedicated Skilled Nursing Facility Physician to join our post-acute care team in California. This role focuses on compassionate, patient-centered care for residents in SNFs and rehab facilities, with emphasis on managing complex, post-acute cases and ensuring smooth transitions of care.

The position requires active CA licensure, board certification in IM or FM (geriatric preferred), and a track record of collaboration with interdisciplinary teams to optimize

Qualifications

• MD/DO from an accredited medical school.
• Board certification in Internal Medicine or Family Medicine; Geriatric Medicine a plus.
• Active and unrestricted medical license in California.
• Experience in post-acute, SNF, long-term care, or geriatric settings.
• Strong clinical skills for complex, multi-morbid patients.
• Excellent communication and teamwork in an interdisciplinary environment.
• Proficiency with electronic medical records (EMR).

Responsibilities

• Provide comprehensive medical assessments and ongoing management for SNF patients.
• Coordinate treatment for patients with multiple chronic illnesses and complex regimens.
• Conduct daily or scheduled rounds in SNFs and respond promptly to acute changes.
• Collaborate with facility staff, nursing, rehab, social work, and case management.
• Communicate with families about goals, plans, and transitions of care.
• Ensure compliance with SNF regulations, documentation, and quality measures.
