Job summary

Sutter Medical Group in Elk Grove, California, is seeking a Skilled Nursing Facility Physician to join our post-acute care team. The role focuses on high-quality, patient-centered care for residents in SNFs, with emphasis on managing complex, multi-morbid patients and coordinating transitions of care.

You will conduct rounds, collaborate with nurses, therapists, social workers, and case managers, document in the EMR, and participate in quality improvement and CMS-compliant reporting.

Qualifications

• MD or DO from an accredited school
• Board certification in Internal Medicine or Family Medicine (geriatrics preferred)
• Active CA medical license
• Experience in post-acute, SNF, long-term care, or geriatric settings
• Strong communication and interdisciplinary collaboration skills

Responsibilities

• Provide medical assessments and ongoing management for SNF patients in post-acute, rehab, and long-term care
• Coordinate treatment for patients with multiple chronic illnesses and complex regimens
• Conduct daily or scheduled rounds in SNFs and monitor acute changes
• Collaborate with facility staff, nursing, rehab, social workers, and case managers
• Communicate goals and care plans with families and teams
• Ensure SNF regulatory compliance and quality measures
• Support smooth transitions of care and reduce readmissions
• Participate in quality improvement and population health initiatives
• Document accurately in EMR per policy and regulation
