WNY’s Preeminent Life Care Community is seeking a compassionate and highly motivated *LPN* or *RN *to join our team on our fifty (50) bed person-centered care CARF accredited Skilled Nursing unit in Williamsville, NY. *Open Position: *
* _*Full Time (5 days/week)*__: _11pm - 7am

*Job tasks include:*
* Participate in the development and implementation of care plans, including quarterly reviews, as they relate to care.
* Prepare, administer and document medications as per procedure.
* Demonstrate to residents and staff personnel, as necessary, the procedures involved in the resident care.
* Encourage residents to perform or participate in ADL functions and range of motion exercises. Assists residents with range of motion as necessary.
* Collect data and report resident concerns to RN/MD or interdisciplinary team as applicable for follow-through. Assure directions and follow-through on unit and completion of documentation.
* Conduct regular walking rounds of nursing unit.

*LPN/RN qualifications: *
* Must possess a current, unencumbered, active license to practice as an LPN or RN in New York State or permit.
* Training in rehabilitative and restorative nursing practices is preferred.
* Must pass a medical evaluation, background check, and drug test.
* Must have reliable transportation.

*LPN Wage Range: *$27.00 - $30.00

*RN Wage Range:* $32.00 - $38.00
