ComForCare Home Health Care (North San Diego) is seeking a compassionate Private Duty Nurse (PDN) to provide high-quality one-on-one nursing care in patients' homes in San Diego, CA.

Immediate start, full-time position with a supportive team culture and flexible scheduling. You will assess health status, administer treatments, and educate families while maintaining thorough documentation in a private duty home health setting.

Qualifications

• Active LVN license in the state of California.
• 2 years of experience or more in private duty nursing/home health.
• Current CPR/BLS certification required.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide direct private duty nursing care to patients in a home health setting.
• Assess, monitor, and document patient health status and progress.
• Administer medications, treatments, and procedures as prescribed.
• Develop and implement individualized patient care plans.
• Communicate with patients, families, and the care team on needs and updates.
• Educate patients and caregivers on health management, medications, and safety.
• Maintain accurate clinical documentation in compliance with regulations.
