Job summary

Meridian Health Services in Wayne, Indiana is seeking a full-time Skill Builder I to join our Children’s services team. You’ll work with therapists to develop and implement individualized plans, delivering both individual and group skills training for adults and children.

As part of a mission-driven team, you’ll coordinate care, complete reassessments, and liaise with external agencies while advancing your professional development in a supportive, growth-focused environment.

Qualifications

• High school diploma or equivalent with 2 years behavioral healthcare experience.
• Associate degree is preferred.
• Valid driver's license and reliable transportation are required.

Responsibilities

We are currently seeking a full-time, day-shift Skill Builder I to join our Children's services team, located in Wayne, Indiana.

About the Role:

As a Skill Builder, you’ll play a vital role in helping patients and families develop essential life skills that support developmental milestones and reduce psychosocial barriers. You’ll work closely with therapists and medical staff to coordinate care and implement individualized treatment plans.

Key Responsibilities:

• Collaborate with therapists to develop, implement, and revise treatment plans.
• Deliver individual and group skills training for adults and children, focusing on:
  • Self-care and daily life management
  • Interpersonal communication and parenting
  • Illness and anger management
  • Positive coping and problem-solving skills
• Provide case management services aligned with treatment goals.
• Support integrated care by coordinating with medical providers and identifying health-related needs.
• Complete CANS/ANSA reassessments and maintain accurate clinical documentation.
• Serve as a liaison between patients and external agencies or institutions.
• Participate in ongoing professional development and training.

What We’re Looking For:

We’re seeking compassionate, motivated individuals who are passionate about whole-person care and thrive in a collaborative, team-based environment. Experience in behavioral health or social services is a plus.

• High school diploma or its equivalent with 2 years behavioral healthcare experience or equivalent college coursework toward a human services or health related degree required; Associate Degree preferred
• Valid driver’s license, a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance required.
