Sickle Cell Foundation of Georgia, Inc. seeks community health workers to promote health and wellness for individuals living with sickle cell in Georgia.

CHWs bridge gaps between care access and community resources, working in homes, schools, clinics, shelters, and centers to support engagement and self-care. Responsibilities include outreach to patients out of care, education and coaching, enrollment assistance for the patient-powered registry, and coordination with clinics and providers.

Formación

• High School Diploma or equivalent and at least one year of college.
• Good understanding of the community and services.
• Knowledge of SCD preferred.
• At least one year of experience in a community setting.
• Excellent communication skills and written communication ability.
• Ability to provide emotional support and motivation.
• Certification training in National Sickle Cell Disease Lay Program.
• Valid driver’s license and reliable transportation.
• Satisfactory driving record and background check.

Responsabilidades

• Provide community-based services reflecting SCD needs, including nutritional and transportation assistance.
• Identify and outreach to persons with SCD who are out of care.
• Establish trusting relationships with persons living with SCD and families.
• Maintain relationships with clinics, hospitals, schools, and community centers.
• Conduct needs assessments to identify barriers to care.
• Provide education, coaching, support, and follow-up for active participation in health care.
• Provide enrollment assistance for the SCD patient-powered registry and update as needed.
• Follow up with clients via phone, home visits, and other settings.
• Communicate with Health Educator to track progress and outcomes.
• Know and utilize community resources; maintain a database of services.
• Collaborate with team and document services in designated software.
