Job summary

NorthStar Services seeks Shared Living Providers to welcome Participants into their homes in Northeast Nebraska. Providers deliver habilitation, medical support, meals, and daily living assistance to persons with intellectual and developmental disabilities.

Responsibilities include documentation, safety, and coordination with NorthStar care teams. Providers receive ongoing training, background checks, and support from NorthStar, including nurse oversight and monthly program reviews to ensure

• Willingness to share home with a Participant
• Ability to document habilitation, medication administration, and medical care
• Ability to provide support with community involvement, meals, personal care, transportation, finances, communication, leisure, and daily routines

• Providing and documenting habilitation, medication administration, medical care and appointments
• Support and assistance with community involvement, healthy and/or therapeutic meals, personal care, transportation, finances, phone/communication, leisure and free-time activities, interpersonal relationships and family roles, reciprocity activities, and establishing or maintaining a daily routine
• Support and assistance with a supportive, safe, and healthy living environment
• Monthly monitoring of the Shared Living setting
• Providing back up staff and ensuring training and background checks are up to date

Shared Living Provider (Private Contractor)

NorthStar Services provides services topeople with intellectual and developmental disabilities throughout NortheastNebraska. NorthStar office locations areBloomfield, Columbus, Fremont, Norfolk, O’Neill, Valentine, Wayne, and WestPoint. NorthStar provides services tothese areas and surrounding areas.

A Shared Living Provider shares their home with a Participant and is responsible for the following services to the participant:

• Providing and documenting habilitation, medication administration, medical care and appointments
• Support and assistance with community involvement, healthy and/or therapeutic meals, personal care, transportation, finances, phone/communication, leisure and free-time activities, interpersonal relationships and family roles, reciprocity activities, and establishing or maintaining a daily routine
• Support and assistance with a supportive, safe, and healthy living environment

NorthStar provides the following servicesto the Shared Living Provider and the Participant:

• AnnualBackground check and associated costs, including One Source, APS/CPS registrychecks and Medicaid Exclusion
• Providingrequired training
• CompletingHCBS reviews
• Assistingin acquiring an NPI# and troubleshooting in Maximus
• Verifyingprovision sheets and entering online for payment
• ProvidingQuality Assurance activities and oversight
• EmergencyAssistance
• Accessto NorthStar Nurse and Nurse oversight
• Writingprograms/Healthcare Staff Objectives/Mental Health Staff Objectives, conductingmonthly reviews on all documentation and handling any necessary revisions
• Monthlymonitoring of the Shared Living setting
• Providingback up staff (staff to fill in for the provider as needed) including all back-groundand training needed
• Providingsupport staff (provides services alongside the provider at the same time)
