Title: Service Coordinator II

Organization: National Church Residences - All Divisions

Status: Non-exempt

Reports to: Property Manager/Portfolio Manager for Enriched Housing Services/Program Manager

Revision date: August 2022

PURPOSE

Under the general supervision of the Property Manager, Staffing Manager, or Program Manager, and according to organizational policies, federal, state, and accreditation standards, the Service Coordinator is responsible for coordinating programs and services to help participants maintain a good quality of life and age in place, prioritizing a person-centric service delivery model.

ESSENTIAL FUNCTIONS AND DUTIES
  1. Program Coordination: Responsible for coordinating programs and activities for eligible individuals, including:
  • Serving as a liaison to community agencies, networking with service providers, and seeking new services. Identify low-cost providers and negotiate discounts.
  • Monitoring service provision from community agencies and documenting participant use.
  • Developing and maintaining a Resource Directory of local and state service providers.
  • Organizing educational events on health care, agency support, life skills, and referrals.
  • Identifying partners for health services and screenings.
  • Assisting participants in building informal support networks.
  • Engaging volunteers per organizational policies.
  • Individual Service Coordination: Providing person-centered assessments, interventions, and monitoring, including:
    • Outreaching to eligible individuals for education and enrollment.
    • Conducting assessments and screenings as per requirements.
    • Collaborating and communicating with care teams.
    • Providing quality customer service, addressing questions and concerns.
    • Helping participants access community services addressing social determinants of health.
    • Following up and monitoring as per participant needs and program requirements.
  • Additional Responsibilities: Performing miscellaneous tasks, building rapport with participants, managing time independently, responding to communications, and maintaining accurate documentation.
  • Documentation: Completing timely electronic and written documentation, including care plans, monitoring, and reports for funding and quality assurance.
  • PERFORMANCE MEASUREMENTS

    Meeting quality assurance, regulatory, and contractual requirements, and aligning with organizational core values.

    EXPECTATIONS

    Maintaining caseload ratios, demonstrating professionalism, adhering to policies, respecting individual rights, completing mandatory training, and complying with all regulations.

    QUALIFICATIONS
    • High School Diploma or GED required; Bachelor's preferred.
    • Minimum of two years in social services, with knowledge of supportive resources.
    • Strong communication and interpersonal skills.
    • Proficiency in Windows, Microsoft Office, and web navigation.
    PHYSICAL ACTIVITIES & REQUIREMENTS

    Travel as needed, with activities involving climbing, stooping, lifting, standing, walking, pulling, sitting, and driving, with specified weight capacities.

    WORKING CONDITIONS

    Indoor and outdoor environments with temperature variations, noise, dust, and other elements.

    MENTAL ACTIVITIES

    Applying reasoning, performing math, and understanding English language requirements.

    Supervision & Conditions

    Occasional supervision, working independently with defined objectives, and acceptable working conditions.
