Work Schedule: Hybrid: 2 Days Onsite, 3 Days Remote
The Service Coordinator works with families located in Chicago's north side neighborhoods, up to and including Evanston.

Pay Range: $20.19 - $31.80 per hour

Summary
The Service Coordinator I with Child and Family Connections assists in determining eligibility for designated services and enables families to receive their rights, procedural safeguards, and services that are authorized under the state’s early intervention program. The Service Coordinator builds relationships that include providing information to families and coordinating information among multiple service providers. The service coordinator maximizes the use of existing resources and refers families to any other needed services, tailoring the unique circumstances of each individual child and family. The Service Coordinator exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.
