Job summary

AIM is seeking a Service Coordinator to support the Nursing Home Transitions and Diversion (NHTD) and Traumatic Brain Injury (TBI) programs. This part-time role offers growth potential to full-time and a full benefits package including medical, dental, vision, life insurance, 403(b) with matching, and generous PTO.

The position involves home visits, community education, and development of service plans while ensuring detailed recordkeeping and time tracking.

Qualifications

• Education: Must hold a Bachelor's degree.
• Experience: At least 3 years of experience providing case management, service coordination, and community-based services.
• Education: Must hold an Associate's degree.
• Experience: At least 5 years of experience providing case management, service coordination, and community-based services.

Responsibilities

• Comply with NY DOH NHTD and TBI service coordination waiver requirements.
• Home visiting and community education.
• Develop, review, and submit service plans.
• Maintain detailed recordkeeping with strict attention to detail and time tracking.

Position Snapshot:

The Service Coordinator will support AIM’s Nursing Home Transitions and Diversion (NHTD) and Traumatic Brain Injury (TBI) team, which provides support and services to people with disabilities and the elderly who wish to continue living in their homes. This is a part-time position with growth potential for full-time.

Essential Functions:

• Comply with all aspects and responsibilities, including the NY State Department of Health (DOH) NHTD and TBI service coordination waiver services.
• Home visiting and community education.
• Develop, review, and submit service plans.
• Maintain orderly recordkeeping with strict attention to detail and articulate time tracking.
• Participate in the development of the programs.
• Case management for consumers participating in services. Up to 25 individuals.

Candidate Requirements:

Bachelor's Degree Pathway

• Education: Must hold a Bachelor's degree.
• Experience Requirement: Must have at least three (3) years of experience providing the case management, service coordination, and community-based services as outlined above.

Associate's Degree Pathway

• Education: Must hold an Associate's degree.
• Experience Requirement: Must have at least five (5) years of experience providing the case management, service coordination, and community-based services as outlined above.
