Job summary

Service Coordination Unlimited Inc is seeking a Service Coordinator I to join our team in the community. You will assess and secure services for participants, support families, and document visits with accuracy, aiming for person-centered outcomes.

The role requires a Bachelor’s degree or 3+ years in social services, strong communication, and the ability to travel across Lackawanna and Luzerne counties. Travel-friendly candidates are encouraged to apply.

Qualifications

• Bachelor’s degree REQUIRED (in social work, psychology, or related fields preferred). Practicum experience is a plus.
• At least three years in a social service or healthcare-related setting REQUIRED.
• Experience with people with disabilities or seniors and/or LTSS, Service Coordination, or Case Management preferred.

Responsibilities

• Assess, identify, secure, and monitor services for participants.
• Respond to participant needs through various communication methods.
• Meet compliance standards and required timeframes for assessments and plans.
• Document visits and contacts in required systems with accuracy.
• Provide training and support to participants and consumer employees.
• Maintain internal databases to track participant information and encounters.
• Assist participants in obtaining accessibility adaptations, equipment, and transportation.
