Sentara Community Care was founded with the goal of developing innovative care models that address and reduce health disparities in under-resourced communities. We are seeking a dedicated individual to serve as an Associate Medical Director/Clinician across our Hampton Roads locations, splitting time between Norfolk and Newport News, VA.

Position Highlights:

  • Location: Newport News and Norfolk, VA
  • Schedule: 2 days a week at each site, 1 day a week admin (Mileage reimbursement included)
  • Experience with licensed provider treating family medicine, pediatrics, and adults with complex issues
  • Shared call schedule, with RN first call
  • Co-manage patients with a diverse care team including RN, Counselors, Care Management, and others to improve health holistically
  • Review quality and outcome metrics monthly with the healthcare team
  • Collaborate with civic and faith-based leaders on community education programs
  • Dedicated to addressing community needs, fostering relationships, and promoting health equity
