Qualifications

• Must hold applicable therapy licensure (PT/OT/SLP) and be in good standing.
• Experience with senior care and multidisciplinary teams preferred.
• Ability to conduct assessments and develop care plans.

Responsibilities

• Provide therapy and wellness services for older adults (PT/OT/SLP).
• Conduct assessments and create personalized care plans.
• Deliver evidence-based interventions as part of a team.
• Support care coordination with check-ins and telehealth.
• Maintain professional and ethical standards.
• Contribute to program development and quality improvement.
• Help residents maximize independence and confidence.
