Job summary
EmpowerMe Wellness in Sarasota, FL is seeking licensed therapists (PT/OT/SLP) to join our EmpowerMe 360 program for older adults. You will assess, design personalized care plans, and deliver evidence-based therapies in collaboration with a multidisciplinary team.
Responsibilities include care coordination, wellness check-ins, and promoting residents' independence and safety. Flexible schedules and a supportive culture are offered to help you grow professionally.
Qualifications
- Licensure as a therapy professional for seniors (PT/PTA/OT/COTA/SLP).
- Ability to assess, plan, and implement therapy for older adults.
- Collaborative approach with multidisciplinary teams.
Responsibilities
- Provide therapy and wellness services for older adults (physical, occupational, or speech).
- Conduct assessments, create personalized care plans, and deliver interventions.
- Support care coordination with wellness check-ins and caregiver communication.
- Maintain professional and ethical standards aligned with initiatives.
- Contribute to program development and quality improvement.
- Help residents maximize independence, confidence, and satisfaction.
